PCAB Survey Data Reveal Progress, Gaps

Recent PCAB (Compounding Pharmacy) Accreditation survey data show meaningful improvement but also highlight opportunities for continued growth. The latest Quality Review edition of Surveyor features the first cohort of compounding pharmacies surveyed under revised standards that align with updated USP requirements.
Ten standards were cited as noncompliant on more than 25% of surveys, with four from Section 6: Provision of Care and Record Management. The most frequently cited standard, TCRX6-C, focuses on safe storage, labeling, handling, and disposal of pharmaceutical components and preparations.
While cited on 41% of surveys, it shows notable progress from three years ago, when 69% of compounding pharmacies in this cohort struggled meeting requirements.
Similarly, the second most cited standard, TCRX6-D, shows improvement in shipping system validations, declining from 63% noncompliance in 2022 to 37% in 2025.
