Advanced Lithotripsy
Initial and renewal certification for licensed organizations with defined service eligibility. Applicable performance data (at least one month for initial certification; 12 consecutive months for renewal) must be submitted at the time of application.
Comprehensive Lithotripsy
Initial and renewal certification for licensed organizations that provide contracted lithotripsy services to multiple providers. Applicable performance data (at least one month for initial certification; 12 consecutive months for renewal) must be submitted at the time of application. These organizations have a plan for continuous participation in research.
