Welcome

Like me, most nurses with years of experience in home care understand the benefit of home-based care to the client and to their family. As a population, we see increasing demands put on families with aging family members and on those with medically fragile infants, children, and adults. Placement in a long-term care setting may not be optimal, preferred, or even available, when home care with the coordinated assistance of family and healthcare providers is a viable option.

ACHC was first established as a program to set standards for in-home aide services. Home Care Accreditation is at the heart of what we do: it’s people helping people.

The program team shares a deep understanding of how home care works and the range of services it can encompass. We offer knowledgeable, accessible, and enthusiastic support to help you get started or maintain the accreditation you’ve already earned.

Home Care Accreditation Services and Distinctions

Services

ACHC Standards—the requirements you must meet to achieve ACHC Home Care Accreditation—are designed to be relevant to the focus of your business model. ACHC will work in partnership with you to build a customized accreditation solution based on your needs. View available services and distinctions below.

Home Care Aide services include hands-on personal care provided by a Nursing Assistant or certified Home Health Aide (HHA).

Home Care Companion/Homemaker services are provided hourly or by shift. These services are “non-hands-on” and do not require a clinician to supervise.

Home Care Nursing services are usually provided either hourly or by shift and are covered by various payors, but not Medicare. Services can be provided by a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Private Duty Nursing services provide a variety of care in a variety of settings. This could include ventilator care, infusion nursing care, and/or wound care.

Home Care Occupational Therapy services are provided by a licensed Occupational Therapist or Certified Occupational Therapy Assistant (COTA), and include upper body strength training, improving range-of-motion skills, and a home exercise program.

Home Care Physical Therapy services are provided by a licensed Physical Therapist (PT) or Physical Therapy Assistant (PTA), and include gait training, strength training, and range-of-motion exercise programs.

Home Care Social Work services are provided by a qualified Social Worker or a Social Worker Assistant under the supervision of a qualified Social Worker. These services include resolving social or emotional problems and assisting with community resource needs.

Home Care Speech Therapy services are provided by a licensed Speech Language Pathologist (SLP) for services such as improving swallowing, cognition, and speaking.

Distinctions

Distinctions offer additional recognition for agencies that offer specialties that go above and beyond basic accreditation requirements. These options must be earned in conjunction with (or after being awarded) Home Care Accreditation by ACHC.

This additional recognition focuses on patient- and family-centered care that optimizes quality of life throughout the continuum of illness by addressing physical, intellectual, emotional, social, and spiritual needs and facilitating patient autonomy, access to information, and choice. ACHC Palliative Care Standards are based on the National Consensus Project Clinical Practice Guidelines for Quality Palliative Care.

This additional recognition focuses on the provision of care to clients/patients with acute or chronic conditions, using telehealth technology that allows monitoring in the clinical or home environment. This technology creates disease management empowerment and independence, improved access to care, increased collaboration among healthcare providers, and improved client/patient outcomes. Standards were based on telehealth clinical guidelines of the American Telemedicine Association.
