Advanced Home-Based Care
In-Home Hospital Care Accreditation
Hospital-level care delivered in the patient’s home.
Putting patient experience first
Many patients, given the choice, would opt for treatment at home, where they feel most comfortable. Home-based care providers and hospitals are innovating (and collaborating) to make this preference a viable reality. This care model promotes improved outcomes, lower cost than inpatient care, and greater patient satisfaction.
With expertise in both hospital and home-based care, ACHC is ideally positioned to validate quality for advanced home-based care through In-Home Hospital Care Accreditation.
Welcome
COVID-19 accelerated interest in “hospital-at-home” by providing proof of concept that home-based care can be a safe, high quality, and efficient solution for many conditions that often default to inpatient admission.
Developing In-Home Hospital Care Accreditation offered the opportunity for ACHC’s acute care and community care divisions to collaborate closely. My colleagues in Home Health Accreditation, in outpatient procedural care programs, and in the hospitals program observed organizations delivering this level of services; drafted, commented on, and revised standards, and reviewed beta-test survey results prior to launch.
As we worked together, it became clear that the eligibility would be broad and based on each applicant organization’s staffing levels, experience in, and commitment to providing home visits at a frequency that supports an effective treatment plan.
We are now ready to offer this unique program to providers—both hospitals outsourcing for in-home care and home-based providers that address high acuity patients—secure in the knowledge that it meets an emerging need for meaningful evaluation of advanced, non-surgical patient care beyond the inpatient setting.
