The Hospice Outcomes and Patient Evaluation (HOPE) tool changes how hospices collect and report patient data, moving from retrospective chart reviews to real-time assessments during care. HOPE collects detailed information on each patient’s needs, symptoms, and preferences at admission, throughout care, and at discharge. This enhanced data will strengthen care quality, compliance, and performance, while requiring hospices to adapt workflows, train staff, and submit records electronically.
