The most frequently cited deficiency in Behavioral Health organizations is tied to security and privacy of protected health information (PHI). Surveyor findings did not indicate that organizations failed to appropriately manage PHI, but that the guiding policies and procedures failed to address the need to communicate policies to clients/service recipients or to obtain confidentiality statements from personnel.
