Required Client/Patient Records

Client/patient records are a vital part of the accreditation survey process. When a Surveyor arrives for your on-site survey, your organization must provide several client/patient record reports during the opening conference.

A new ACHC checklist is available to help you assess your organization’s compliance with client/patient record requirements.

Required Client/Patient Records

Your Surveyor should be able to view or access all required client/patient records. Client/patient records should be available for the Surveyor to access either in hard copy or electronic format.

Being prepared to promptly supply client/patient record reports is critical. Failure to grant the Surveyor timely access to client/patient records may lead to survey deficiencies.

The Surveyor will use the reports to determine which client/patient records will be reviewed and which client/patient home visit will be conducted during your survey.

Three client/patient record reports are required:

  • Active Census Report
  • Schedule of Visits Report
  • Discharged/Transferred Clients/Patients Report
