Patient records are a vital part of the accreditation survey process. When a Surveyor arrives for your on-site survey, your home health, hospice, palliative care, or renal dialysis organization must provide several patient record reports during the opening conference. A new ACHC checklist (see details further down) is available to help you assess your organization’s compliance with patient record requirements.

Reports required for the following settings:

Required Patient Records

Your Surveyor should be able to view or access all required patient records. Patient records should be available for the Surveyor to access either in hard copy or electronic format.

Being prepared to promptly supply patient record reports is critical. Failure to grant the Surveyor timely access to patient medical records may lead to survey deficiencies.

The Surveyor will use the reports to determine which patient records will be reviewed and which patient home visits will be conducted during your survey.

Four patient record reports are required:

  • Unduplicated Admissions Report
  • Active Census Report
  • Schedule of Visits Report
  • Discharged/Transferred Patients Report
